What do I need to bring to my appointment?

If you are being certified for WIC, you will need to bring:

  • A current Medicaid card 
  • Pay check stubs for everyone in the home 
  • Proof of your physical address. (If your physical address is different than your mailing address, please bring proof of both.)
  • Current proof of ID for you, and your child. Your child's ID can be insurance card, social security card, or for newborns, the Mother's Verification of Facts.
