Medication Assistance Program

Eligibility for medication assistance includes patients who are age 18-64, who are uninsured and whose family income is at or below 200% of the Federal Poverty Limit. Case management services may include but are not limited to:

  • Provide guidance in completing and submitting the Medication Assistance Program application
  • Connecting patients with information for affordable health insurance/Medicaid
  • Connecting clients with appointments for primary care and other doctor’s visit needs
  • Assisting patients locate the resources they need for any request they may have related to their health care or daily life improvement
  • Lab work ordered by patient’s primary care provider
  • Any services available to patient covered by the Graham County Department of Public Health’s standing orders
  • Referrals to Graham County Department of Public Health program services
  • The primary care provider is responsible for assessments and writing of all prescriptions, lab orders and referrals to any specialty clinics.
